To be eligible for a nomination by an individual or an organization, the person must, at the time of the submission:
The person or the organization, submitting the nomination, must duly complete the electronic application form.
For a nomination made posthumously, only the applications submitted within 30 months after the death will be considered.
All Québec residents and organizations may submit one or more nominations no later than February 1st of each year, by completing the online form for each person recommended.
This online procedure is mandatory. In order to dedicate the adequate time to process the submissions, the form will not be available online after the deadline. Additionally, please note that no extension will be granted to transmit the application form, no matter what the reason.
When completing the form, you must consider the following:
The medal will be presented to the recipients during a number of award ceremonies presided by the Lieutenant Governor.
Upon receipt of an official letter of invitation from the Chief of Staff and Private Secretary of the Office of the Lieutenant Governor, each recipient must attend, in person, the ceremony to which she or he was invited in order to receive the medal. A recipient cannot be represented otherwise than posthumously.
